This page explains what happens to the information you share when you use the inquiry form on this site.
What is collected
The inquiry form is hosted by Jotform. When you submit it, Jotform receives the answers you provide — typically your name, email, and short written responses about what brought you to the site.
How it is used
Submissions are used only to respond to your inquiry and, if we begin working together, to coordinate sessions. Information is not sold, shared for marketing, or used for any purpose unrelated to your care.
Where it lives
Submissions are stored by Jotform and reviewed by Blake Rupert. Jotform’s own privacy practices apply to data held on their servers.
How long it is kept
Inquiry responses are kept while your inquiry is active or while we are working together. If you do not become a client, your submission is deleted within twelve months.
Your rights
You can ask at any time to see, correct, or delete what has been collected about you. Write to blake@blakerupert.com and your request will be honoured.
