This page explains what happens to the information you share through the October interest form or by emailing the practice.

What is collected

The October interest form is hosted by Jotform. When you submit it, Jotform receives the answers you provide: your name, email address, city and province or territory, and preferred session format.

The form does not ask for health or clinical information. Please do not include that information in any field. If you are in immediate danger or crisis, use emergency or crisis services rather than this form.

How it is used

Submissions are used to understand interest in possible new-client appointments beginning in October 2026 and, if capacity can be confirmed, to contact you by email. Submitting the form does not reserve an appointment or establish a clinical relationship. Information is not sold, shared for marketing, or used for advertising.

Third-party services

The website itself does not store interest-form submissions, run analytics, or set marketing cookies. Interest-form submissions are processed by Jotform, the third-party form provider. Jotform and its service providers may process or store this information outside Canada, including in the United States, where it may be subject to the laws of those places.

How it is protected

The public website is served over HTTPS. Access to interest-form responses is limited to Blake Rupert and the service providers needed to operate the form and related practice systems. Information is only shared where needed to manage October interest, contact you if availability can be confirmed, comply with legal obligations, or address a serious safety concern.

How long it is kept

Interest-form responses are reviewed and deleted when they are no longer needed. When information is used to make a decision that directly affects you, it is retained for at least one year after that decision, as required by British Columbia’s Personal Information Protection Act, and then deleted promptly when it is no longer needed.

If you later become a client, only information relevant to your care is transferred into the approved clinical record. The redundant Jotform copy is deleted when operationally and professionally permitted. The practice reviews retained interest-form submissions at least monthly.

Your rights and complaints

You can ask at any time to see, correct, or delete what has been collected about you, subject to any legal or professional limits that apply. You can also raise a privacy concern or complaint by writing to blake@blakerupert.com. The concern will be reviewed and answered as directly as possible.
